Voluntary participation and risks

I voluntarily take part in the Silent Grounds Collective (SGC) West Cemetery cleanup on October 24 and/or 25, 2026, from 10 a.m. to 4 p.m., at 250 Hanover Street, Meriden, Connecticut. Outdoor cleanup may involve uneven or slippery ground, holes, unstable monuments, sharp debris, brush and thorns, insects and ticks, weather, lifting, repetitive work, hand tools and other volunteers. These conditions can cause injury, illness, property damage or death. I understand these risks and voluntarily assume the risks of my participation to the extent permitted by law.

Safe work and preservation

I will check in with SGC, follow the site leader’s instructions, use suitable footwear, gloves and other required protection, and accept only tasks I can perform safely. I may decline a task or stop at any time and will report hazards, injuries and unsafe conditions. I will not move, lift, reset or repair monuments; excavate; apply chemicals; or use power tools or heavy equipment without specific authorization and appropriate training. Bringing a tool does not authorize its use. I will respect graves, visitors and historic materials.

Limited release of liability

To the fullest extent permitted by Connecticut law, I release Silent Grounds Collective and its officers, directors, employees and volunteers from claims I may have for personal injury, illness, death or property damage arising from my voluntary participation, including claims based on their ordinary negligence only where such a release is legally enforceable. This agreement does not release gross negligence, reckless or willful misconduct, intentional wrongdoing, or any liability or right that cannot lawfully be released. It is not a promise that all claims are waived.

Minors and guardian consent

Volunteers of all ages are welcome to take part in age-appropriate tasks. For a volunteer under 18, a parent or legal guardian must complete and sign this form, give permission for the child to participate, and accompany and supervise the child throughout the event. The guardian confirms their authority, agrees to the safety rules and discusses the risks with the child. Any release by the guardian applies only to the guardian’s own claims to the extent allowed by law; it does not waive a child’s rights that cannot lawfully be waived.

Emergency assistance and personal information

If an emergency occurs, I authorize SGC to contact the listed emergency contact and request emergency assistance. This does not authorize non-emergency medical treatment or guarantee medical coverage. I understand that SGC uses the registration details for event coordination, safety and a record of this agreement. Responses are emailed to SGC and stored privately in its WordPress account; they are not posted publicly. Please do not include diagnoses, identity documents or other sensitive medical information.

Electronic signature

I have read and understand this agreement, have had the opportunity to ask questions, and choose to sign it electronically. I am at least 18 and sign either for myself or as the parent/legal guardian identified in this form. My typed full legal name and checked agreement box are intended as my electronic signature. If any provision is unenforceable, the remaining provisions apply only to the extent permitted by law. Connecticut law applies.
